How We Collect Information About You: ACTS and CSA collect data through a variety of means including but not necessarily limited to letters, phone calls, emails, voice mails, and from member and client data entered directly on the CSA website. The submission of member and client information is necessary to access services from ACTS or to process the data entered for analysis as part of the CSA web based application.
What We Do Not Do With Your Information: Client information that you provide to us in writing, via email, on the phone (including information left on voice mails), contained in or attached to referrals, requests for services, or entered electronically on the CSA website, is held in strictest confidence.
We do not give out, exchange, barter, rent, sell, lend, or disseminate any information about CSA members or ACTS clients who apply for or actually receive any of our services. This information is considered confidential, is restricted by law, or has been specifically restricted by a patient/client in a signed HIPAA consent form.
How We Do Use Your Information: Information is only used as is reasonably necessary to process requests for client services or to provide members access to the CSA web application. Private client information is maintained in the ACTS office in a secure location and is available only to ACTS’ Associates and administrators who are responsible for providing service and processing billing information. Private client information (name, date of birth, address, phone) entered on CSA is encrypted and visible only to the CSA member who entered that data.
Providing clinical services through ACTS requires communication between other health care and service providers, public and private funding agencies, and insurance companies as necessary to: verify your client information is accurate; determine the type of services that are required and to obtain authorization for any type of educationally or medically necessary equipment that is recommended by our associates.
If you apply or attempt to apply to receive services through us and provide information with the intent or purpose of fraud or that results in either an actual crime of fraud for any reason including willful or un-willful acts of negligence whether intended or not, or in any way demonstrates or indicates attempted fraud, your non-medical information can be given to legal authorities including police, investigators, courts, and/or attorneys or other legal professionals, as well as any other information as permitted by law.
Information We Do Not Collect: The CSA site uses temporary session cookies to allow users to interact with the portal and to use online features. Session cookies do not allow us to personally identify a user. They are stored only in memory and are deleted when the user's browser is shut down.
Limited Right to Use Non-Identifying Personal Information: Any pictures, stories, letters, biographies, correspondence, or thank you notes sent to us become the exclusive property of ACTS. We reserve the right to use non-identifying information about our members and clients (those who receive services or goods from or through us) for promotional purposes that are directly related to our practice.
Non-personal client information such as medical and speech/language diagnosis and data entered on CSA may be used for a variety of purposes such as further analysis of the integrity of the tool, and clinical research to evaluate client profiles associated with medical and speech diagnoses.
Clients will not be compensated for use of this information and no identifying information (photos, addresses, phone numbers, contact information, last names or uniquely identifiable names) will be used without client’s express advance permission.
You may specifically request that NO information be used whatsoever for promotional purposes, but you must identify any requested restrictions in writing. We respect your right to privacy and assure you no identifying information or photos that you send to us will ever be publicly used without your direct or indirect consent.
